Short-term rehab can feel confusing and fast-moving—this program helps you understand what really happens and how to be prepared once you or your loved one are there.
This program provides a practical, non-clinical overview of short-term rehabilitation in nursing homes, focusing on what the experience is like and how to navigate it once a stay begins. While the session will briefly touch on planning ahead, the primary focus is on understanding the process in real time and knowing how to respond as things develop.
Topics include what to expect during a short-term rehabilitation stay, how day-to-day decisions can impact the overall process, discharge considerations, and how and when to involve the Ombudsman as a resource for advocacy and support.
This program is presented by Theresa Norbom, NYS Certified Ombudsman, Regional Program Director of Hudson Valley LTC Ombudsman Program.
